2011 National Healthcare Quality and Disparities Reports

The National Healthcare Quality Report (NHQR) is a comprehensive national overview of quality of health care in the United States. It is organized around four dimensions of quality of care: effectiveness, patient safety, timeliness, and patient centeredness.

Consistent with the Agency for Healthcare Research and Quality (AHRQ) Prevention Quality Indicators (PQI) software, chronic obstructive pulmonary disease must be the principal diagnosis.� Transfers from other institutions are excluded. Rates prior to 2005 are not reported because of International Classification of Diseases, 9th Revision (ICD-9-CM) coding changes.

^b. Rates are adjusted by age and gender using the total U.S. resident population for 2000 as the U.S. standard population.
