Asthma Health Disparities Collaborative Coalition Guide

Michigan's approach to facilitate implementation of the Asthma Health Disparities Collaborative is decentralized and uses its existing infrastructure of regional and local asthma coalitions to serve as the primary liaison with the federally qualified health centers (FQHCs). The Asthma Initiative of Michigan identifies asthma coalitions to participate in Rhode Island Health Disparities Collaborative training. Then the asthma coalitions and FQHCs take part in an Asthma Initiative of Michigan Health Disparities Collaborative learning lab on May 22, 2007; this is followed by three Plan-Do-Study-Act cycles. Between PDSA cycles, the coalitions and FQHCs meet to review data and plan the next PDSA cycle. At the conclusion is a sharing successes session, or outcomes congress.
