Appendix Figure C-2 (Text Description)

This 4-page form is used to create a detailed abstract for each article. It includes article ID, author, and reviewer. The user completes the form by checking boxes under Perspective (e.g., clinician, hospital), Data Source (e.g., primary), and Time Frame (e.g., longitudinal/panel). There are also boxes to enter the sample size and year range. A series of Yes/No questions are included regarding comparison and use of measures. The user also completes a table about the measures. At the top, the user enters the total number of measures. For each measure, the user enters the measure number and checks boxes under columns for inputs, outputs, explanatory variables, methodology, type of efficiency measured, and reliability/validity.
