Readmissions are supposed to be counted in the numerators according to the measure specifications, but were not counted because the data were not available at this time.

^b Per measure specifications revised 6/22/2005, percents and standard errors are calculated based on data weighted to reflect the contribution of each State's sample to national estimates.
