Project RED: Re-Engineered Discharge (Staff Training)

Provides a four-module training program to help hospitals implement Project RED.

Complete the Patient Care Plan

  • Medication reconciliation
  • Pending tests and results
  • Post-discharge services
  • Primary care provider
  • Follow-up appointments
  • Information about condition

Notes:

"Once the discharge order is written, the DA finalizes the Patient Care Plan and ensures that all sections are complete, accurate, and in terms and language that the patient will understand. This slide lists the elements of the Patient Care Plan."
