"Other therapists may also be involved in determining post-discharge services or providing patient education. For example, physical therapy, occupational therapy, or speech therapy may be part of the patient's health care team. The hospital's disease management program should also be included it on the project team to ensure that patient education is coordinated as well. The admitting department and emergency departments also will be important for evaluating the results of Project RED."
