In the Swim Lane Diagram of the discharge process, we are concentrating on the:
- Physician writing the discharge order.
- Nursing staff educating the patient and assisting with the discharge process and the patient's departure.
- DA completing the Patient Care Plan, instructing the patient on all aspects of the plan, and arranging for the post-discharge phone call.
- Pharmacist assisting with medication reconciliation and, perhaps, teaching and following up with the patient.
