Teaching and Teach-Back

- All education material
- Care plan completed
- Two printed copies
- Copy to quality department
- Meet in quiet place
- Review all parts of the Patient Care Plan
- Confirm understanding using teach-back methods
Notes:
When all the educational material is collected and the Patient Care Plan is completed, the DA prints the plan for the patient and conducts the final teaching. A second copy might be given to your quality department to review its completeness as one of your metrics.
The DA meets with the patient and family in a quiet place to review teachings that occurred during the patient's stay and to teach any new information that resulted from final discharge orders. The DA covers all parts of the care plan and assesses the patient's understanding.
