- Information should flow from the medical record to the care provider who needs it.
- Information should flow from one practice setting to another.
- Information that is documented can be time stamped and assessed for accuracy.
- The discharge care plan could be automated and flow to the hands of the care team and patient.

Creating and modifying the Patient Care Plan for the unique needs of your patients will be important. While we cannot always obtain the ideal state of EMRs and forms that flow seamlessly throughout the system, it will be important for you to consider the elimination of redundancy and rework that is placed upon nurses in their daily routines. We recommend that your IT department be involved in the solution that generates and monitors the Patient Care Plan. During a pilot test of Project RED, you may have to tolerate more manual or key entry time. Once you have decided to spread Project RED beyond a small group of targeted patients, IT solutions may be built.
