Provides a four-module training program to help hospitals implement Project RED.

Notes:

It's helpful for the team to have a group appreciation of potential and actual causes of failure in their discharge process. You have addressed your various re-admission rates specific to your patient population. A Cause and Effect Tool is used with this module so the team can generate a list of potential causes of system breakdown that result in patient readmissions for your hospital.

Solutions that are generated will only be successful and sustainable if they are directed at the elimination of root causes.
