Your Current State May Look Like This

This is an example of a hospital's discharge process. It reflects the reality of the discharge order being written on the night before discharge, and the work being done in a flurry by the next morning's discharge hour. Why do we need to look at our current state process by using a high-level process map? Without designing a high-level process map, team members are unable to visualize what actually happens in the eyes of the patient. Once the team draws the current state process, potential failures and risk points can be determined.
