We encourage AHRQ to reserve some level of funding for demonstration/pilot programs to develop communty-based effectiveness review mechanisms. These pilots would test models of practice review at the local level that incorporate best practice standards and local variation data that support recommendations for change targeted at local medical communities. Pilots associated with emerging RHIOs will allow the development of trusted mechanisms that, in time, will draw on clinical data from health information exchanges.
