Clinicians and patients who work as a team to make shared decisions are part of a delivery system model focused on high-quality health care and improved outcomes. The following free print resources from AHRQ can help you work with your patients to communicate effectively and make shared decisions. These publications are being made available as AHRQ transitions from printed materials to Web-based resources.

Measuring Your Blood Pressure at Home (PDF, 564 KB)
This brochure helps educate patients with hypertension and explains how to properly take a blood pressure reading at home.

Reducing the Risk of Bone Fracture (PDF, 394 KB)
This brochure explains how low bone density can increase the risk of breaking a bone and what research has found about different treatments to lower the risk. It can help patients talk with doctors about treatment options.

Questions Are the Answer Waiting Room Video 
This seven-minute DVD features engaging examples of patients and clinicians discussing the importance of asking questions at medical visits. It can run on a continuous loop in your waiting area.

This consumer guide can be used by nurses and discharge planners to help patients care for themselves after their hospital stay. This booklet comes in a bilingual (English/Spanish) flipbook format.
