Impact Case Study: Gurwin Jewish Nursing Home and Rehabilitation Center

  • On-Time Quality Improvement in Long-Term Care:
    • Reduced incidence of pressure ulcers by 70%.
    • Integrated four On-Time reports to identify residents at risk for pressure ulcers.
    • Targeted residents at risk for unintended weight loss; incidence fell 3.5% to 1.7%.
    • Documentation of patient behavior increased from 35% to 85%.
