  • Health IT offers enormous benefits, but carries risks to patient safety as well.
  • In response to public interest and hearings by both ONC [the Office of the National Coordinator for Health IT ] and IOM [Institute of Medicine], AHRQ enhanced its ability to capture information on Health IT-related patient safety events through its Common Formats.
  • By working collaboratively with FDA [Food and Drug Administration], AHRQ developed a Common Format that will guide reporting of such events both to PSOs [Patient Safety Organizations] and to FDA's adverse event reporting system MAUDE [Manufacturer and User Facility Device Experience].
