Project Description: The objective of this application was to develop collaborative management of Type 2 Diabetes in Black Americans. The research was accomplished by investigating the patient, provider, and systems level barriers to the effective management of diabetes in primary care settings and developing a collaborative model of care to potentially eliminate these barriers. The research had three specific goals:

1. Identify the knowledge, beliefs, and attitudes of Black patients that impact their diabetes outcomes.
2. Identify impediments to improved diabetes outcomes in Blacks that are attributable to health care providers.
3. Identify external factors that impact on diabetes outcomes in Blacks.

Career Goals: Dr. Egede is an Associate Professor of Medicine in the Division of General and Internal Medicine and Geriatrics at the Medical University of South Carolina in Charleston and Director of the MUSC Center for Health Disparities Research. He received his MBBS in Medicine/Surgery from the University of Benin, Nigeria and did his residency in Internal Medicine at the Greater Baltimore Medical Center. Dr. Egede continues to investigate effective interventions to improve patient-provider communications and reduce racial/ethnic differences in health outcomes in patients with type 2 diabetes.

Progress to Date: This grant has been completed. Dr. Egede found African Americans had poorer diabetes-specific knowledge, poorer diabetes self-management skills, and felt less empowered to care for diabetes compared to Whites.
