  • But the pace is slow, especially for:
    • Preventive care
    • Chronic disease management

This slide contains a bar graph that shows the number of measures that showed improvement for core measures, process measures, outcome measures, and total of all measures combined. For core measures, two-thirds showed improvement (22 of 33 measures), 66 percent of process measures improved (45 of 68 measures), 63 percent of outcome measures improved (58 of 92 measures), and 65 percent of all measures combined improved (110 of 169 measures).
