  • Develop a data-driven quality improvement model for Medicaid recipients with chronic disease.
  • Provide for a comprehensive electronic health record with clinical decision support.
  • Connect Health and Human Service Agency data.

*Patient 1st is the foundation

Notes: The grant requirements have been simplified to three goals we use in presentations and in any publications. All of this is being done using our Primary Care Case Management (PCCM) program, Patient 1st, as the foundation, which allows us to target the providers for participation and provides us with the leverage to enter into contracts for purpose of the pilot.
