Use of AHRQ's Preventable Hospitalization Costs and Mapping Tool

The left-hand column of the chart is titled HEDIS 2009 TN Health Plan Average vs. HEDIS 2009 National Averages. The middle column is titled Commercial HMO and is separated into two subcolumns. The left subcolumn is titled HEDIS 2009 Tennessee and the right is titled HEDIS 2009 National. The third column is titled Medicaid HMO and is separated into two subcolumns. The left subcolumn is titled HEDIS 2009 Tennessee and the right is titled HEDIS 2009 National. Rates are shown for HbA1c control, cholesterol control, and blood pressure control for patients with diabetes, blood pressure control for patients with hypertension, and cholesterol control for patients with cardiovascular conditions. Tennessee's commercial HMO rates are below the national rate for every measure except blood pressure control in patients with diabetes with blood pressure less than 140 over 90. Tennessee's Medicaid HMO rates are better than the national rates for HbA1c and blood pressure control in patients with diabetes (blood pressure less than 130 over 80) and for cholesterol control in patients with cardiovascular conditions. The other rates are below the national rate.
