Health Information Exchange

The slide is a graphic representation of health information exchange between individual care providers, data sources and public health. There are 3 levels:
At the Find level, Individual care providers contact a patient index. Provider asks if there are records for his/her patient. Index sends location of any records. The patient index and data sources connect through locator. Source sends index information.
At the Get level, individual care providers contact a data sources. Provider asks for and receives records. Data sources send records to provider through message transfer.
At the Send level, data sources report data to public health through a reporting router where data is de-identified. Source may push data for reporting.
