iCare: Population Management GUI

iCare is a customizable GUI that provides clinicians with the functionality that will facilitate caring for a group or population of patients.
There are 4 views of patients in iCare: the patient view, the provider view consisting of multiple patients in a panel, population views defined by identifying taxonomies of data elements, and community views consisting of population data and specific community data
Users can define their own population of patients based upon primary provider, case manager, as well as a host of additional characteristics such as gender, age, diagnosis, community, or other documented data element. In this example, a patient panel named Diabetic Teens is being created by searching for patients age 13-19 years with a diagnosis of diabetes.
