The PWH is a new tool that is designed to provide patients with information from their medical record utilizes recommendations from the Institute of Medicine and a program developed by the Agency for Healthcare Research and Quality called putting prevention into practice.

The first version of the PWH provides the patient with: immunizations due, BMI, blood pressure, allergies, and current medications
