Ultimate Proposed Data Linkage/Sharing Scheme
A flowchart illustrates the data linkage. First there is common anchor data for CSHCN. This is linked to two data bases. The first includes Electronic birth certificate and vital records, immunizations, WIC, EPSDT and lead screening. This links to the TN Department of Health, NBS and NHS and then the data flow to the TN-CHP Comprehensive Child Health Profile. The second database includes Tennessee Early Intervention, and TennCare Mental Retardation. This data flows into genetic and child development centers and Volunteer e-health Initiative, and then the data flow to the TN-CHP Comprehensive Child Health Profile.
