- Two or more eligible patient visits with PCP
- Initialization of Lists: PCP can report:
- "Not my patient" or "Not Diabetic"
- Conflicts adjudicated (<.1%)
- Weekly Updating:
- New patients
- Transfers within or across practices
- No complaints
