Performance data: Event reports to understand and implement changes in systems of care. Use of event data as baseline and remeasurement to assess impact.
Falls collaborative, 11 hospitals, 2 long term care facilities.
Provided falls data for six months prior RCA training and six months post to monitor any change in falls, initial or repeat.
